What is Transition?

School transition is about a student’s preparation and moving from school to work and community life.

How does it work?

It consists of working together with school staff, family or whanau and a transition consultant to gather enough information to help the student identify his or her goals upon leaving school, and what strategies need to be put in place to achieve these goals. This is called a Transition Plan.

Assistance may include providing information to help you decide, visits to workplaces, work experience, mentoring and career planning.

This should assist the student to move easily from school into paid work, training or further education, and community integration. The support provided by the Transition Consultant continues after the student leaves school.

It’s a client driven and very flexible service, so it is very important that the student makes the consultant aware of his wishes throughout the process.

For more information on this service please contact us or download our Transition Referral Form. This form needs to be completed by your key contact at school.
